Schools must report number of student athletes each school year. Students should only be reported one time per school year no matter how many sports they are involved in.

This form is PER SCHOOL. There can be NO COUNTY/SYSTEM WIDE ENROLLMENTS. Please also check with others in your school system to make sure there are no duplicate enrollments.
